Q1. Bindu, thank you for taking the time to speak with us. You operate at a “billion-claim scale,” where data decisions directly affect millions of lives. When you think about this level of impact, how do you personally define what meaningful success looks like in healthcare data engineering today?

Bindu Madhavi Mangalampalli: Meaningful success, for me, is never just about processing throughput or uptime; it’s about the downstream human impact of every pipeline I build. When you’re operating at a billion-claim scale, as I do at Cotiviti, every data decision you make ripples outward. A misclassified diagnosis code, a delayed risk score, a broken ETL… these aren’t just technical errors. They can affect a patient’s coverage, a provider’s reimbursement, or a payer’s ability to allocate resources where they’re truly needed.

So I define success on three levels. First, the technical soundness of the systems must be accurate, scalable, and compliant, particularly under HIPAA. Second, decision enablement (the insights we produce) must be actionable, not just available. If a clinician or a payment integrity analyst can’t act on the data in time, the best architecture in the world has failed its purpose. Third, and most importantly, lives are better served when our risk adjustment models catch a chronic condition that was undercoded, or when our fraud detection flags an anomaly before a payer loses millions. Real people benefit on both ends of that equation.

With 17+ years in data engineering across healthcare, manufacturing, and telecom, I’ve learned that success at scale is deeply relational; it’s about ensuring that the millions of records I touch every day represent real patients who deserve accurate, timely, and fair healthcare decisions.

Q2. In your paper, “Scalable Data Warehousing Techniques for Healthcare Analytics,” you discuss the importance of designing systems that can handle both volume and variability in healthcare data. From your experience, what’s something people often overlook when they think about “scalability” in real-world healthcare systems?

Bindu Madhavi Mangalampalli: The word “scalability” tends to be reduced to a single dimension in most conversations: volume. People ask, “Can your system handle 10 million records? A billion claims?” And yes, that’s important. But in my paper “Scalable Data Warehousing Techniques for Healthcare Analytics,” I deliberately focused on another dimension that is often ignored: variability.

Healthcare data is not clean, uniform, or predictable. You’re ingesting claims from hundreds of different payers, each with its own formats. You’re mapping ICD codes that shift with every version update. You’re reconciling lab values in different units across different EHR systems. Real scalability means your architecture can absorb that structural chaos without degrading quality or requiring constant manual intervention.

At Cotiviti, I’ve architected data platforms that handle not just millions of records but millions of different kinds of records (clinical, financial, pharmacy, lab), all of which need to be harmonized into a single decision-ready view. The systems that break under pressure are almost never the ones that ran out of compute. They’re the ones that were built assuming uniform, predictable data structures.

My approach has always been to design for schema flexibility and robust data governance from day one, not as an afterthought. The Oracle data warehouse architectures and AWS-based data lakes I’ve built embed data quality checks directly into the ETL pipeline, so variability is handled gracefully rather than reactively. That, in my view, is what true scalability looks like in healthcare.

Q3. In your work across 7+ payment integrity products at Cotiviti, you’ve been involved in transforming complex clinical and claims data into decision-ready intelligence. Could you share a moment where connecting seemingly unrelated data points led to a breakthrough insight for payer operations?

Bindu Madhavi Mangalampalli: One of the most impactful moments in my work at Cotiviti came when I was involved in a payment integrity initiative where we were analyzing claims across multiple product lines. On the surface, the datasets seemed completely unrelated: pharmacy claims from one source, professional service claims from another, and facility billing records from a third.

What we discovered by building a unified data model that joined these streams was a pattern of temporally inconsistent billing situations in which a patient was billed for inpatient facility care on the same dates they had outpatient pharmacy fills at retail locations. Independently, neither anomaly would have been flagged. But connected, they pointed to a systematic billing discrepancy worth significant recovery dollars for our payer clients.

What made this possible was the investment we had made in building a cohesive enterprise data warehouse that treated every domain, clinical, pharmacy, and financial, as part of one integrated picture rather than siloed data marts. The insight wasn’t brilliant data science; it was the discipline of building the right foundation so that connections could be made.

That experience reinforced something I believe deeply: in healthcare analytics, the most powerful discoveries rarely come from a single data source. They emerge at the intersection of domains, which is exactly why I advocate for integrated architectures over fragmented, product-specific data systems. Across the 7+ payment integrity products I’ve been involved in at Cotiviti, this philosophy of connected intelligence has consistently driven our most meaningful operational outcomes.

Q4. Your research on “Optimizing Risk Adjustment Models Using BI Platforms in Healthcare Systems” touches on improving financial and clinical outcomes. How do you approach aligning technical optimization with the human realities of patient care and provider workflows?

Bindu Madhavi Mangalampalli: This is a tension I think about constantly, and my research on “Optimizing Risk Adjustment Models Using BI Platforms in Healthcare Systems” was born partly from that discomfort. Risk adjustment is a domain where the math has to be right, but “right” isn’t purely statistical. It has to reflect clinical reality.

My approach starts with stakeholder immersion before code. Before I optimize any model or pipeline, I try to deeply understand the workflows of the people who will use or be affected by the outputs, whether that’s a clinical coder reviewing a suspect diagnosis, a physician being asked to close a care gap, or a payer analyst making coverage decisions. Technical optimization that ignores those workflows tends to produce outputs that are accurate in a vacuum but ignored in practice.

One concrete example: when I was improving our ETL processes at Cotiviti, ultimately achieving a 30% improvement in reporting efficiency, part of the work involved redesigning the output layer of our dashboards. The original design was technically correct but required analysts to navigate four levels of drill-down to access the data they needed daily. By restructuring the data model to surface the most clinically relevant risk indicators at the top level, we didn’t just make the system faster; we made it usable. Adoption went up, which meant the insights actually influenced decisions.

I also believe strongly in HIPAA-aware design as a form of trust infrastructure. When providers and patients know their data is handled with integrity and compliance baked in, not bolted on, they engage more openly with data-driven care decisions. Technical excellence and human trust are not in conflict; they’re mutually reinforcing. 

Q5. You’ve also explored real-time analytics through your work on dynamic BI frameworks. How has the expectation for “speed” in data changed the kinds of questions stakeholders are asking today?

Bindu Madhavi Mangalampalli: The shift has been profound, and it’s not just about dashboards refreshing faster. The expectation for real-time or near-real-time analytics has fundamentally changed the nature of the questions stakeholders ask.

Five years ago, a payer executive might ask: “What was our claims volume last quarter?” That’s a retrospective question, valuable, but backward-looking. Today, thanks to the dynamic BI frameworks and real-time analytics architectures I’ve been involved in building, those same stakeholders are asking: “Which high-risk members haven’t had a qualifying encounter in the last 30 days, and can we intervene before the month closes?” That’s a prospective, actionable question, and it requires a fundamentally different data infrastructure to support.

My patent concept around a Dynamic BI Dashboard Framework for Real-Time Healthcare Analytics was born precisely from this shift. The goal wasn’t speed for its own sake. It enabled a class of questions that simply couldn’t exist in a batch-processing world: questions about the current state, predictive risk, and moment-to-moment course-correction.

The challenge this creates for data engineers like me is significant. Real-time systems are harder to build reliably, govern, and reconcile with healthcare’s strict compliance requirements. But they’re also where the highest-value decisions get made. A risk flag arriving three weeks after the month-end is interesting. A risk flag that arrives while a care coordinator can still act on it is transformative.

The stakeholders who are asking the best questions today are the ones we’ve succeeded in educating and empowering through better data experiences. That feedback loop, better systems enabling better questions enabling better systems, is one of the most exciting dynamics in healthcare analytics right now.

Q6. As someone actively involved in AI/ML research and also leading production-scale implementations, where do you currently see the most underexplored opportunity for AI in healthcare analytics, something the industry hasn’t fully tapped into yet but holds significant potential?

Bindu Madhavi Mangalampalli: If I had to point to one area that I believe is significantly underinvested relative to its potential, it would be AI-driven data quality and intelligent data reconciliation, specifically within the context of healthcare claims and clinical data pipelines.

The industry has rightly gotten excited about AI for diagnosis prediction, drug discovery, and clinical decision support. But the unglamorous, foundational problem that limits all of those applications is data quality at the source. In my 7.5+ years in healthcare data engineering, I’ve seen brilliant AI models fail not because the algorithms were wrong, but because the training data was riddled with coding inconsistencies, duplicate records, missing values, and format mismatches that no one had systematically addressed.

The opportunity I see is in building AI-powered, self-healing data pipeline systems that can detect anomalies in incoming data, infer likely corrections based on historical patterns, flag ambiguous records for human review, and continuously learn from downstream corrections. This is different from traditional data validation rules, which are static and reactive. Intelligent pipelines would be adaptive and proactive.

My pending patent on an Automated Healthcare Data Aggregation and Risk Scoring system touches on this space: the idea that ETL pipelines should not just move data, but understand it well enough to catch what a human reviewer would catch, at machine speed and scale.

The second underexplored area I’d highlight is AI for longitudinal patient journey modeling using machine learning across multi-year claims histories to predict not just risk scores but also intervention windows. Most risk adjustment today is annual and retrospective. AI could make it continuous and prospective. The data exists. The compute is accessible. What’s missing is the architecture and the organizational will to build it, and that, I believe, is the frontier where the next decade of healthcare AI value will be created.
